STUDENT WAIVER
I/we realize that participation in dance classes and activities could involve some possible personal injury. Despite precautions, accidents and injuries may occur. By signing this release form, I/we (_________________________) assume all risks related to the use of any, and all, spaces used by KC Social Dance Studio, LLC DBA Viva Social Dance Studio. I/we agree to release and hold harmless KC Social Dance Studio, LLC DBA Viva Social Dance Studio including its independent teachers, dancers, staff members, apprentices, and facilities used by both entities from any cause of action, claims, or demands now and in the future. I/we will not hold KC Social Dance Studio, LLC DBA Viva Social Dance Studio liable for any personal injury or any persona property damage, which may occur on the premises before, during or after classes. Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by KC Social Dance Studio, LLC DBA Viva Social Dance Studio. In case that I damage any part of the studio, I agree to cover the cost of replacement of Studio furniture, mirrors, or any part of the facility.
CANCELLATION POLICY
Drop-in Visits: If you are unable to attend, please inform us at least two hours before 5:00 PM CST in order to save your purchase for future use. No refunds are allowed for drop-in visits or any other purchase made at the front desk, via the app or online.
Memberships: When you purchase our membership program, you are agree to a minimum of four months with recurring auto billing. After the initial four months, you are billed monthly and any cancellation must be done with a 30-Day written notice prior to the next billing cycle. The billing cycle begins on the original date of purchase. There are no exceptions to this policy. The cancellation policy is subject to change, and any updates will be communicated to our customers immediately via email and posted on our website at VivaSocialDanceStudio.com.
Class Card: Class cards have an expiration date of 1-year from date of purchase.
